Items you will need:

• Background Check Consent Form, completed
• Driver's License or other government ID

Please note: The Background Check Consent form, a scan of your driver's license or ID, and the Volunteer Application must ALL be submitted together. We are unable to accept or process volunteer applications without these required documents.

Instructions:

1. Download the Background Check Consent form. You can complete it digitally or, if preferred, print it, fill it out manually, and scan it for upload.
2. Once the form is completed, upload it to the "Upload Background Check Consent Form" field in the Volunteer Application below.
3. Next, scan your driver’s license or government-issued ID and upload it to the "Upload Driver's License or Government ID" field in the application form.
4. Finally, complete the Volunteer Application form and submit.
